OUR SERVICES • Sick Care • Well-Child Physicals • Height & Weight • BMI & Blood Pressure • Immunizations • Vaccine record check • Lead & Hemoglobin screens (for those entering Head Start) PROGRAM OVERVIEW St. Joseph’s Children’s Mobile Medical Clinic (MMC) is a pediatric office on wheels that travels to undeserved communities in Hillsborough County to provide healthcare services. We have specialized in well-child care and will now be adding a sick care component as part of a partnership with Metropolitan Ministries. SCOPE OF SERVICES - WELL CHILD CARE _IMMUNIZATIONS_ We administer all vaccines recommended by the ACIP. _HEARING & VISION SCREENS_ Referrals provided for children identified with hearing and/or vision concerns. _VACCINE RECORD CHECK_ Vaccine records put into the Florida Shots™ database. Parents are given a certified vaccine record. DO YOU QUALIFY FOR MEDICAL SERVICES? Children age 18 years old and younger Children must have : -Medicaid or - No Health Insurance or - Be Native American Photo ID for parent or guardian Child’s most recent immunization records Registered Participants must first check in at the Family Support Center Front Desk.
