FOUR POINT PLAY - COVID 19 PROCEDURES ENTERING PROGRAMMING
1.Facilitator will take temperature of each participant entering into
the gym.
2. Facilitator will ask a series of questions to participant or
participant's parent/guardian:
a. Have you had a cough, fever and/or trouble breathing in the last
couple of days?
b. Have you tested positive for COVID-19?
c. Have you had close contact with anyone that tested positive for
COVID-19?
(If the answer is yes to any of the above questions, we can not
proceed with in-person programming with the participant. If the answer
is no, proceed to number 3.)
3. Participant and facilitator must sanitize their hands before
entering into the gym.
4. We will not allow any materials brought from home into the gym. We
will provide all necessary materials needed for training.
EXITING PROGRAMMING
Participant must discard any materials utilized during programming
(i.e. water bottles, food etc.)
Participant must place materials (i.e. basketball, towel) utilized
during programming in "UN-SANITIZED SECTION"
Facilitator and participant will sanitize their hands before exiting
the gym.
Four Point Play presents Raising Athletes Open Gym! Our professionally
trained certified coaches will supervise participants with warm-up and
scrimmage with their peers in a fun and safe atmosphere. Time to get
your kids back on the court and ready to have some fun!
Participants: BOYS & GIRLS, Grades 6th-8th
LIMIT 20 PARTICIPANTS PER SESSION
LOCATION: Walther Christian Academy, 900 Chicago Ave, Melrose Park, IL
Sessions will happen as follow:
SUNDAY - OCTOBER 11TH: 1:30PM-3:30PM
SUNDAY - OCTOBER 18TH: 1:30PM-3:30PM
SUNDAY - OCTOBER 25TH: 1:30PM-3:30PM
SUNDAY - NOVEMBER 1ST:1:30PM-3:30PM
NOTE: SESSIONS ARE HELD IN THE GYM LOCATED IN THE BACK BUILDING OF
WALTHER CHRISTIAN ACADEMY. CROSS STREETS ARE SUPERIOR AND 9TH AVE.
