The purpose of this symposium is to raise awareness of important updates in guideline directed therapies to optimize the management of Heart Failure. This program is designed to meet the educational needs of physicians and all health care providers who participate in the care and management of patients at risk for, or diagnosed with Heart Failure.

Learning Objectives

1. Identify updates to guideline directed therapies appropriate to stage classification of heart failure.
2. Identify interdisciplinary resources available to reduce heart failure readmissions.
3. Review key components of strategies to prevent readmissions.
4. Identify indications and appropriate referrals for advanced heart failure therapy
5. Discuss disparities in the diagnosis, care and management of heart failure.
6. Review the role of a heart failure clinic in post-acute care management.
7. Identify various surgical and device therapies in heart failure treatment.
8. Explore advanced and emerging therapies in heart failure management.
