This is a Food Safety Preventive Controls Alliance (FSPCA) class put
on by ISU/CIRAS/ANR. It is the second part that is required after
the first part online is completed in order to receive your
certificate as a Preventive Controls Qualified Individual (PCQI).
The FSPCA PCQI class is the only PCQI class recognized by the FDA.
Regulations require that companies have a PCQI as set forth by the
FDA.To successfully complete part 2, you must:
* Part 1: Online be completed before attending Part 2 of the BLENDED
CLASS AND COMPLETION TICKET MUST BE SENT TO kandrson@iastate.edu
before the start of class.
* Must bring FSPCA HUMAN FOOD PARTICIPANT MANUAL VERSION 1.2. (You

* Attend in person for the entire class.
* Bring Proof of Identification. (EX: Driver's License or another
Official Government form of ID)
* Participate.

In light of the current situation, we will be offering the Part 2:
Instructor-led Human food PCQI class virtually. In order to do this,
the Food Safety Preventive Controls Alliance (FSPCA), the approving
global organization of this course, requires the following:
* 8 contact hours where breaks and lunch time aren’t included,
thus, requiring the following schedule
* Check-in time at 7:45 AM
* Class start time at 8:00 AM
* Class end time at 5:00 PM
* Attendees must have internet and computer capabilities
* Attendees must be able to attend and be actively present during
the course for the full amount of time using the format determined by
the instructor. (We will be using WebEx as our format.)
* A FSPCA Human Food Manual is REQUIRED for this course.

* Attendees must agree to being recorded during the class as this is
required by FSPCA as proof of class instruction.
* All attendees are required to provide governmental identification
(state driver’s license or passport) as proof of identity to
instructor
* Part 1 must be submitted to INSTRUCTOR (Kimberly L Anderson at
kandrson@iastate.edu) seven days before Part 2 class.
* Active participation is required, meaning participants must be
able to type private chat responses to the instructor, Kimberly L.
Anderson during the class, be able to speak oral responses through
audio AND video capabilities.
* Attendees camera must stay on during the whole course instruction
period.
* Missing or delayed responses to questions, exercises, and
activities is considered an absence and the class fees are forfeited
with no certificate from the instructor.
* Course must be in full attendance with video, audio, and type
responses to actively prove attendance.
* _Successful active participation, full instruction attendance, and
correct real-time responses are required to successfully complete the
Part 2: Instructor-led Human Food PCQI course._
* __If this cannot be done ISU will refund money for event if
notified BEFORE APRIL 24, 2020.
INSTRUCTOR INFORMATION:
Kimberly L. Anderson is a Lead Instructor for the FSPCA Preventive
Controls for Human Food Course. She comes for a diverse background
in the food industry and was a former FDA Investigator.
