FREE TO BE ME: TRANSITION PLANNING FOR YOUTH: MEDICAL HOME,
POST SECONDARY EDUCATION; EMPLOYMENT OPTIONS, ESTATE PLANNING,
ALTERNATIVES TO GUARDIANSHIP, SCHOOL ISSUES
• Free To Be Me is a transition-focused workshop for young adults
with disabilities (12 years+) and their parent(s)/guardian(s). The
parent(s)/guardian(s) and young adult attend the learning sessions
together to develop a transition plan which includes the school
transition process, life goals, employment goals, self-determination
skills and self advocacy development. Parent(s)/guardian(s) will also
learn about alternatives to guardianship and how to encourage and
support their young adult’s independence.
