The Finger Lakes Performing Provider System (FLPPS) is a partnership comprised of 19 hospitals, 6,700 healthcare providers and more than 600 healthcare and community-based organizations in a 13 county region (Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Steuben, Wayne, Wyoming and Yates counties).FLPPS Partners collectively serve over 300,000 Medicaid beneficiaries and 100,000 uninsured individuals throughout the Finger Lakes region. --> FLPPS is divided into five geographic sub-regions, termed Naturally Occurring Care Networks (NOCN). These Networks represent the full continuum of care and organizational leadership within a shared geographic service area.  Each NOCN is led by a participant workgroup that represents the healthcare providers and community based organizations in their area.  The FLPPS Central office is the Team that supports the FLPPS Partnership in building an integrated healthcare delivery system (IDS) to better support Medicaid and uninsured populations across the Finger Lakes region. The FLPPS Central office provides Partners with Delivery System Reform Incentive Payment (DSRIP) program implementation support through project management, Information Technology (IT) development and integration, Patient Centered Medical Home (PCMH) preparation, Workforce development, Cultural Competency and Health Literacy guidance and incentive payments. FLPPS is governed by a Board of Directors and advised by an Executive Steering Committee. Both groups are comprised of leadership from our regional health systems, FLPPS operational committees, NOCN workgroups and consumer representatives.
